[City of Bloomington, Illinois] Non-Profit Liquor Application

Non-Profit Liquor License Application

Thank you for your interest in obtaining a Non-Profit Liquor license in the City of Bloomington!

This application should be used for civic, charitable, fraternal, educational, patriotic, and/or religious organizations on premises in nonresidential locations for a maximum period of 60 days with the following conditions:

Only civic, charitable, fraternal, educational, patriotic, and/or religious organizations which have been in active and continuous existence for a period of one year prior to the date of making the application or those which are incorporated under the laws of the State of Illinois will be eligible
No organization shall be issued more than three licenses in any twelve-month period. The license shall only apply to catered functions or special events sponsored by the organization.

Please be advised, in order for a submission to be considered complete, the application must be completely filled out and all supplemental attachments submitted by attaching them to the submitted application.

Application Process:*

Applications and all supporting documents must be submitted at least 15 days in advance of the event date.

Once an application and all required supplemental documents are submitted, the review of the application will begin.
Following the review, a City Clerk Department staff member will reach out to discuss the next steps.

Application Supplemental Documentation:

In order for this application to be considered, Applicants must submit the following:

If Applicant will use a different name with the public than the name of the legal entity, proof of the DBA/Assumed Name is registered with the State of Illinois or the County where the Establishment is located.
Food/Beverage Tax Registration Form must be submitted with the City of Bloomington Finance Department at https://cityblm.seamlessdocs.com/f/FoodandBeverageTax
Certificate of Insurance (Not Policy Declaration Page)
1. The Certificate must show the establishment has liquor liability insurance and must include the following:
2. 1. Applicant’s name as the insured (e.g. if the applicant is a corporation, the corporation’s name must be listed);
  2. The date, times, and address of the location where the liquor is being consumed;
  3. The dates of coverage and the coverage limits
  4. Certificate Holder must be listed as:
  5. 1. City of Bloomington, 115 E. Washington St., Bloomington, IL 61701
    2. 1. The City must also be listed as an Additional Insured
Surety Bond in the amount of $2,000 and must include:
1. Applicant's name/entity name and address; and
2. The City of Bloomington, 115 E. Washington St., Bloomington IL 61701 listed as the Obligee.

Conditional Requirements:

NOTE: BASSET Certificates for those selling/serving must be available upon request at the event.

Associated Fees:

$100 for the first licensed day and $50 per subsequent day, up to $500 total.
