  • An updated license/certificate from Illinois Department of Child & Family Services for the licensed premises unless already supplied.
  • An annual inspection to be completed by a City of Bloomington Community Development Inspector. If your establishment has not had its licensing inspection completed, please reach out to Community Development at (309) 434-2226 to schedule an appointment.
  • For Daycare Centers Only (Not Home Daycares): A McLean County Health Department Inspection Report
  • Corporations and Limited Liability Companies Only: Certificate (or full-page print including date) of Good Standing from the Secretary of State’s office: provide a copy of the certificate or a screen shot of the Secretary of State’s website indicating an active status.
