Primary Liquor License Application

This application should only be submitted by businesses looking to apply for a new primary liquor license. If you already hold a primary license and are looking to submit a renewal or secondary license application, please use the appropriate forms located on the City Clerk Department’s License & Permit page.

Before you start this application, please look over the Primary Liquor License Document Checklist at https://www.bloomingtonil.gov/home/showpublisheddocument/32627/639160085734630000. This will help you make sure you have all the required documents ready to upload to this application. You can attach documents at any time using the paperclip icon on the left side of your screen.

Required Documents (must be complete & attached to this application)

Copy of a Government-Issued Photo IDs for all Applicants, Owners, Partners, Officers, etc. with over 20% interest and any General Managers of the Business.(Must be combined into one document for submission)
Financial Statement
- The Statement must show the assets, liabilities, and capital, if any, for all Applicants, Owners, Partners, Officers, etc. with over 20% interest
Premises Verification (Lease Agreement) – If the business location is owned, we will check internally, so no documents are required to be submitted.
Articles of Incorporation (Corporations) or Articles of Organization (LLCs)
Computer-Generated Site/Floor Plan
- The plan must be drawn to scale and must:
- - Show exits, entrances, restrooms, and any other identifying features of the premises;
  - Be labeled to show where alcohol will be stored and sold/served; and
  - Include the ENTIRE premises you wish to be included in the Liquor License, if approved.

Required to be Submitted Online (NOT attached to this application)

Tax Forms – Must be Submitted Online. Local Tax Information can be found on the City’s website at https://www.bloomingtonil.gov/departments/finance/local-tax-information.
- Food/Beverage Tax – Required for all liquor licensees and can be submitted online
- Packaged Liquor Tax – Required for licensees who apply for Packaged (P) alcohol
- Amusement Tax – Required for establishments that will offer events where amusement is exhibitive in nature, meaning it is viewed in exchange for payment of a fee or charge.
- - Includes Cover Charges or Tickets sold
