  • Completed Business/Property Owner form if placed on their property or in front of their business. Please see the License Renewal page to download the agreement form.
  • If food is sold: A copy of the McLean County Food Handler’s Permit
  • Corporations and Limited Liability Companies Only: Certificate (or full-page print including date) of Good Standing from the Secretary of State’s office: provide a copy of the certificate or a screen shot of the Secretary of State’s website indicating an active status.
