The following document(s) are required in addition to the affidavit:
  • An annual inspection to be completed by the Bloomington Police Department. Please reach out to Officer Brace at (309) 434-2467 to schedule an appointment.
  • The financial status of the applicant, including a statement of assets and liabilities; financial information shall also include the amount of all unpaid judgments against the applicant and the nature of the transactions or acts giving rise to said judgments.
  • A letter outlining the experience of the applicant in the transportation of passengers.
  • An updated Certificate of Insurance (Not Policy Declaration Page) for all licensed vehicles. The Certificate must include the following: (1) applicant’s name as the insured (i.e., if the applicant is a corporation, the corporation’s name must be listed); (2) the business address; (3) the dates of coverage and the coverage limits; and (4) list of all insured vehicles including identifying factors (e.g. VIN number, license plate, etc.).
  • Any facts which the applicant believes tend to prove that public convenience and necessity would benefit by the granting of a certificate to the applicant.
  • Corporations and Limited Liability Companies Only: Certificate (or full-page print including date) of Good Standing from the Secretary of State’s office: provide a copy of the certificate or a screen shot of the Secretary of State’s website indicating an active status. (Website: https://www.ilsos.gov/corporatellc/)
  • Application Only: Proof that the business name has been properly registered with McLean County as a DBA or with the Illinois Secretary of State as an assumed name.
IMPORTANT: Owner(s) who will also be drivers must apply for a Taxi Driver Permit.
