Secondary Premises Liquor License Application
Thank you for your interest in obtaining a Secondary Premises Liquor license in the City of Bloomington!
This application should be used by the current holder of an EA/EB, TA/TB, RA/RB, or PA/PB license at nonresidential public premises other than the premises covered by the existing license at locations and days approved by the Liquor Commissioner.

Please be advised, in order for a submission to be considered complete, the application must be completely filled out and all supplemental attachments submitted by attaching them to the submitted application.

Application Process:
Applications and all supporting documents must be submitted at least 15 days in advance of the event date.
  1. Once an application and all required supplemental documents are submitted, the review of the application will begin.
  2. Following the review, a City Clerk Department staff member will reach out to discuss the next steps.
Application Supplemental Documentation: In order for this application to be considered, Applicants must submit the following:
  1. Certificate of Insurance (Not Policy Declaration Page)
    1. The Certificate must show the establishment has liquor liability insurance and must include the following:
      1. Applicant’s name as the insured (e.g. if the applicant is a corporation, the corporation’s name must be listed);
      2. The name, date, times, and address of the location where the liquor is being consumed;
      3. The dates of coverage and the coverage limits
      4. Certificate Holder must be listed as:
        1. City of Bloomington, 115 E. Washington St., Bloomington, IL 61701
        2. The City must also be listed as an Additional Insured
          1. Additional Insured can be listed in the Description box
Requirements Checked Internally
The applicant does not need to submit these, but this information will be checked internally.
  1. Applicant's Primary Liquor License must be active and in good standing with the City and State
  2. The Legal Entity must be in Good Standing with the State of IL (if applicable)
Conditional Requirements:
  1. Special Event Permit (if applicable)
    1. Applicants should visit the City's Special Events Webpage to see if a permit is required.
NOTE: BASSET Certificates for those selling/serving must be available upon request at the event.

Associated Fees:
$100 for the first licensed day and $50 per subsequent day, up to $500 total
